Our Boy Scout Troop uses a two-sided "Camping Permission Slip" and "Equipment List" (samples below).  The parents keep the top of  the permission form with specific information about the campout and general information about camping, and they return the "coupon" at the bottom with their signature.  After the campout, this slip (which includes the payment information) is given to the Troop Treasurer along with the receipts.

On the reverse side (recently we have been printing this as a separate page) is an Equipment List which each Scout (ideally) uses to check off his equipment for each-and-every campout.  The Equipment List changes a little each season, but the basic idea is pack the same basic core equipment every month, rather than deciding on what clothes to pack based on the weekend weather forecast.
